I don't use them. There is no doubt in their minds that they are being broadcast. Their participation is their consent.
Are the laws the same for Internet broadcasting as they are for broadcasting a telephone call on terrestrial radio? Who knows, but I doubt it.
Someone can always think of a reason to sue.
If they are under age... better get a release of some kind.
Participation on your program constitutes implied consent to use your guest’s voice or likeness on the air. If you plan on using the content to sell a product or service beyond the scope of the originally intended broadcast you should speak with an attorney and perhaps have some releases on hand.

We use a release when ever we have a celebrity appear one our shows. We dont use it all the time but if we feel that someone might be a "problem" we make sure to have them sign it
There is a pretty slick iPad app called "Easy Forms". They fill out and sign, you take their picture, then send the form off.
